OKLAHOMA STATE ELECTION BOARD
These Absentee Ballot Application Forms may be used only by residents of the State of Oklahoma.
There are two different absentee ballot application forms, each with different requirements and
instructions. The one you must use is determined by your own personal circumstances, as described
below. Please read the following information carefully and select the correct application form.
If you meet any of the following conditions...
- You are physically incapacitated and you ARE confined to a nursing home or convalescent hospital
- You are physically incapacitated but you are NOT confined to a nursing home or convalescent hospital
- You are charged with the care of another person who is physically incapacitated and who cannot be left unattended
...then you must use
this application.
If you do NOT meet any of the three conditions stated above, you must use
this application.
The instructions on page 2 of each downloadable application form are different.
Please make sure to follow the instructions for the application you select.
Your completed absentee ballot application must be submitted to the county election board in
the county where you are registered to vote. Get county election board addresses and telephone numbers
here.
Attention Military and Overseas Voters
Special absentee ballot application provisions apply to military and overseas voters.
Oklahomans who are in the military or who are living overseas and their spouses and
dependents may use the Federal Post Card Application, available from the
Federal Voting Assistance Program.
